We welcome anyone eligible to register with The New Mill Street Surgery
To determine if you live in our catchment area please check the map below.
Our catchment area is roughly the Thames to the north, Tower Bridge Road to the west, Abbey Street to the south, and Bevington Street to the east.
If you are unsure if you live in this area, please call our reception desk to enquire. Should you not be able to register at here NMS our staff will gladly try to help you find another GP in the area.
If you are eligible you’ll now need to fill out our registration form.
Please fill these confidential forms thoroughly, including all current contact details, (especially your mobile phone number and email address if you have one), don’t forget your NHS number, and make sure you sign the documents. When you enter information about your family’s health history also include any information you know about your grandparents’ health histories.
Once these forms are filled out please bring them to the surgery together with current proof-of-residence so we can process your registration on the computer. We process all registrations after 11 am on the day and it may take up to 48 hours to process your registration on our computer system.
It is vital that you please bring ONE of the following as current proof-of-residence. If unsure about your proof of residence, ask Reception before attending the Surgery.
- Bank Statement
- Utility Bill
- Rent Book
- Lease Agreement
- And a copy of your passport (or other photo ID if not available)
For female patients: If you have a copy of your most recent smear results please bring a copy of these (especially if it was done outside the UK.)
To register with us you can pick up a New Patient Welcome Pack from reception. Alternatively, you can start the registration process here online by downloading and filling out the Registration Form, which needs to be brought to the surgery.
All registrations are dealt with between 11 and 7 pm on Monday, Tuesday and between 11 and 6pm on Wednesday, Thursday and Friday.

Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Registration Policy
In accordance with current recommendations, all new patients should present two forms of identification. This is required to reduce and prevent fraudulent attempts to obtain or misuse NHS care. Overseas visitors may be asked for additional information to prove NHS entitlement.
One form of identity must ideally be photographic such as:
- Passport
- Drivers Licence
- Official ID card from Public Services body
- Student matriculation card (current year)
Other documents for prove of residency that are acceptable are:
- Recent utility bill (within last 3 months)
- Council Tax document
- Television Licence
- Payslips (last two months)
- Rent book/agreement (Public Body or Private Landlord)
- Bank Statement (Name and address section only required)
- Solicitors Letter- (Clearly showing name and address)
If unable to provide photographic evidence then you must present one of the above and one of the following:-
- Birth Certificate
- Marriage Certificate
- DivorceAnnulment Papers.
N.B. The above list is not exhaustive and staff are entitled to ask for further proof if required. Please note you will not be registered with the practice if you cannot provide the documents required to prove NHS eligibility.
Proof of address is not required for patients with no fix abode.
New Patient Health Checks
Once you have completed all the forms and your registration is made on the computer please make an appointment for your New Patient Health Check.
Call NMS at 020 7252 1817 or drop by Reception, whichever is most convenient.
Why do you need a Health Check? It’s important that you undergo a New Patient Health Check as we need to fully understand your past health history, your family’s health history, and what your current health issues might be. To ensure you receive great health care – and to ensure our team delivers exactly that – NMS needs to understand you and your health needs.
When you arrive for your appointment you will be asked to provide a urine sample. Next, you’ll meet with a NMS Clinician for your Health Check to review your health needs and history.
Some simple measurements like height, weight and blood pressure may be taken as well. This should last about 15-20 minutes. The New Patient Health Checks are different to NHS Health Checks which we also offer to all eligible Patients (aged 40-75 without any pre-existing cardiovascular conditions).
Once your Health Check is completed? You’ve joined the NMS family! Now you can fully access the many services on offer at the surgery – you can book that appointment you need, learn about smoking cessation, get travel advice, or even get involved with our New Mill Street Patient Participation Group (NMSPPG)!
Change of Contact Details
Patients registered at NMS are asked to help us by informing us whenever you have a:
- Change of address
- Change in name
- Change of home / mobile phone numbers
- Change of email address
- Change in next of kin details
Please email us at [email protected] if you change your contact details. If you move outside of the catchment area you will be asked to find another GP. We will notify you by letter.
