How to Register
We welcome anyone eligible to register with The New Mill Street Surgery. You can either register on-line or complete a paper registration form (this is also available at reception).
- We don’t ask for any proof of residency, but to register as a regular patient you need to reside in our catchment area or plan to reside for at least 3 months
- We only ask for photo proof of ID so we can enable your online services. You can verify your ID if you register using our online service and login using your NHS app login, if you do that, we will register you for online services automatically as part of your registration process
- Registration will need to be completed before you can book an appointment with us
- We will send a text/call you to confirm you are registered with us
- Please allow five working days from submitting the form for registration to be completed.
You will remain registered with your previous GP until you are fully registered with us. We cannot issue any repeat prescriptions until we receive confirmation of your past medical history and medication so do ask your previous GP for a supply.
If you are unsure on how to register, please ask at reception and they can help you to complete the registration process.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. You may be asked to see/speak to a GP or see one of our nursing staff to complete your registration.
You will be allocated a named GP on registration, but you will be registered with the practice rather than that individual GP. You may see any of the doctors.

To determine if you live in our catchment area please check the map below. You should only send the online registration form if you are sure that you are eligible to join this practice – check practice catchment area.
Our catchment area is roughly the Thames to the north, Tower Bridge Road to the west, Abbey Street to the south, and Bevington Street to the east.
Register online
The NHS registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.
Register with a paper form
If you prefer not to register online please complete the following steps:
- Please check our map (above) to ensure you live in the catchment area
- Download and complete the GP patient registration form (also available from reception)
- Return your completed registration forms to reception or email or post to us
If you don’t have access to emails or a printer and would like us to post a registration to you please give us a call on 020 7252 1817.

If you are not certain if you live in this area, please call our reception desk to enquire. Should you not be able to register at here The New Mill Street Surgery our staff will gladly try to help you find another GP in the area.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

The National Care Record Service (NCRS)
Important information regarding your NHS medical records – Summary Care Record
The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Organ Donation
