Changes to Urgent Appointments from 1st April

From 1st April 2025, we will no longer offer same-day urgent appointments at New Mill Street Surgery without clinical triage, due to service demand. We are introducing an urgent total triage system. Patients requesting an urgent appointment must provide information relating to their urgent health concern using our online form.

How to Register

We welcome all people regardless of their ethnic origin, gender, age, ability, class, religion, sexual orientation or immigration status. You can either register on-line or download and complete a paper registration form below. These forms are also available at reception.

Safe Surgery

We are a Safe Surgery. This means that anyone who resides within our catchment area can register with us regardless of immigration status, language or lack of identification (ID) or proof of address.

In partnership with Doctors of the World UK, we will ensure that our practice offers a welcoming space for everyone who seeks to use our services.

If you wish to register, please speak to a member of staff who can help and support you with the process of registration. Ask reception for a interpreter if you find it difficult to communicate in English.

  • We don’t ask for any proof of residency, but to register as a regular patient you need to reside in our catchment area or plan to reside for at least 3 months. 
  • We may ask for proof of ID so we can enable your Online Services. If you are able to verify your ID using your NHS app login, we will automatically register you for online services as part of the registration process.
  • Registration will need to be completed before you can book an appointment or order repeat medication with us. 
  • We will send a text/call you to confirm you are registered with us. To receive messages from the practice you must use a UK mobile or landline telephone number.
  • Please allow five working days from submitting the form for registration to be completed. 

If you are unsure on how to register, please ask at reception and they can help you to complete the registration process.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. You may be asked to see/speak to a GP or see one of our nursing staff to complete your registration.  

You will be allocated a Named GP on registration, but you will be registered with the practice rather than that individual GP. You can choose to see any of the doctors.

To determine if you live in our catchment area please check the map below. You should only send the online registration form if you are sure that you are eligible to join this practice – check practice catchment area. 

Our catchment area is roughly the Thames to the north, Tower Bridge Road to the west, Abbey Street to the south, and Bevington Street to the east.

The NHS registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

If you prefer not to register online please complete the following steps:

  • Please check our map (above) to ensure you live in the catchment area
  • Download and complete the GP PRF1 Patient Registration Form and Health Questionnaire (also available from reception)
  • To help us process your registration please enter your NHS number if you were previously registered with another practice.
  • Return your completed registration forms to reception or email or post to us

PRF1 Registration Form

Register Temporarily

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary patient for up to 3 months or as a permanent patient.

    No Fixed Abode

    You can register with us if you are homeless or of no fixed abode but reside within the practice boundary. Please contact the practice if you have no fixed abode before completing an online registration

      International Patients

      Please note that whilst Primary Care is available under the NHS, there may be further charges for secondary care if you do not have a BRP/Tier-4 Card. 

      Make your choice about sharing data from your health records

      There’s no deadline for opting out of sharing your confidential patient information for research and planning. You can make or change your choice at any time. For More Information about opting out of NHS services see our Privacy Policy

      Medications

      You will remain registered with your previous GP until you are fully registered with us. We cannot issue any repeat prescriptions until we receive confirmation of your past medical history and medication. If you can not wait to be registered please ask your previous GP for a supply of medication. 

      Benzodiazepines and Opiates

      You should provide official proof of repeat medications, e.g. a reorder slip from your previous practice. It is our policy to review medication and offer dose reduction where appropriate to ensure effective and safe prescribing, particularly for drugs that can be harmful or addictive. These include Diazepam, Gabapentin, Pregabalin, Lorazepam, Temazepam and Tramadol. By registering with us you are agreeing to work with us to make sure your medication is appropriate for you.

      Medical Records Outside the UK

      Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes

      Organ Donation

      From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.

      Outside catchment area

      If you are not certain if you live in this area, please call our reception desk to enquire. Should you not be able to register at here The New Mill Street Surgery our staff will gladly try to help you find another GP in the area.

      There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

      The National Care Record Service (NCRS)

      Important information regarding your NHS medical records – Summary Care Record

      The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
